General Questions
How do I sign up for my membership?
Fill out the contact form above or call our office to schedule your initial appointment and set up your membership.
What can I expect at my first visit?
We are excited to have you as our patient! On your first visit, pay the one-time enrollment fee and first monthly payment and receive the included benefits of the plan you choose. Most plans include a comprehensive exam, x-rays, and a profession cleaning.
What should I do if I have periodontal disease?
Our Perio Plan is designed for you. It includes your regular perio maintenance visits as well as additional discounts on specific perio services. During your initial visit, we will let you know if these services are needed.
What forms of Payment are accepted?
Credit and Debit cards including MasterCard, Visa, American Express, and Discover are accepted.
What is a Consistent Care visit?
A Consistent Care visit includes your preventative regular care of a doctor exam, x-rays, (as needed) and a professional cleaning. Depending on the plan you choose, it may also include additional benefits such as bleaching, oral cancer screening, or fluoride.
How do I schedule an appointment for my included care?

What is a Consistency Reward?
To encourage you to use your included preventative benefits, we will reward you with a $25-100 reward that you can use toward any services you may need. Rewards accumulate after each Consistent Care visit and don’t expire as long as your membership remains active.
Can I combine my Consistency Rewards with other family members?
Yes! Individual rewards may be combined with other family members to maximize savings. Take advantage of having your entire family on the plan by using everyone’s rewards on a single procedure.
What is a loyalty discount?
Loyalty discounts are discounts on any restorative work that needs to be completed, (filling, root canal, crown, etc.) or on any elective work you may desire (implant, veneer, bleaching etc.). This saves you money when work is needed.
How do I use a loyalty discount?
Just schedule the work you need and we will automatically apply the discounts. There is no paperwork or hassle to receive the valuable discounts.
Is there a waiting period for benefits and services?
No! There are no limitations or waiting periods for current or future dental needs.
How do I cancel my membership?
We plan on your membership continuing on a month-to-month basis for as long as you have teeth. The total monthly amount will be billed for a minimum of 6 months and is not cancellable prior. If you need to cancel after the initial 6 months give us a call and we will be happy to discontinue your plan.
